Owing to severe respiratory distress and hypotension (40/25 mmHg), the baby was mechanically ventilated and treated with vasoactive drugs. The O2 saturation was maintained between 25% and 55%, and the pupillary response to light was weak. Parents accepted the withdrawal of life-sustaining therapy (WLST) and agreed to kidney DCD on day 2 of life. Fifteen minutes after WLST, circulatory arrest determined by mechanical asystole occurred. Death was declared following 5 minutes of mechanical asystole.
Case 2
The female preterm neonate was delivered at 29 weeks and 5 days’ gestation by emergency cesarean section owing to intrauterine distress. Her mother had chronic hypertension and preeclampsia. The neonate presented with respiratory distress, hypotonia, and cyanosis at birth and developed generalized seizures on day 1 of life. Apgar scores at 1 minute and 5 minutes were 3 and 4, respectively. Cerebral magnetic resonance images showed intracranial hemorrhage in the right frontal lobe and cerebellum and generalized cerebral edema. The parents agreed to kidney DCD 3 days following birth. Twenty-five minutes after WLST, circulatory arrest determined by mechanical asystole occurred. Death was declared following 5 minutes of mechanical asystole.
